School Health Clinic
Providing primary and urgent care services to the students & faculty of all 26 schools within Madison County.
School Clinic Location: Jackson Careers and Technology Elementary School - 668 Lexington Avenue, Jackson, TN 38301 with a separate entrance located under the blue awning.
• Hours of Operation: Monday – Friday, 7am-3pm. Closed on Saturday & Sunday, and Jackson-Madison County School holidays & breaks.
• To Schedule an Appointment: Please call the clinic front desk at (731) 256-9691 or (731) 256-9692 or sign up for MyChart to conveniently schedule an appointment online.
• Each parent/guardian is required to complete consent and privacy forms prior to receiving medical care. They can be completed by filling out and submitting the electronic form in MyChart. This will go straight to the school clinic staff to upload into the patient’s electronic medical record. The forms can also be completed by walking into the school health clinic or going to the school front office to request the forms.
• Who will see my child? Ashley Hardee is a board-certified Family Nurse Practitioner who will see all students, staff members, and family of staff members. Ashley received her Masters of Science degree in nursing and has an extensive background in primary and urgent care settings.
